Those circumstances include grinding Explorers, Battleships, Surveyors, Borg Probes, Exchange Transports, and mixed environments where you're hitting at least one of these. There are exceptions.

Compare it's stats before and after combat, you'll see it becomes way stronger (hull jumps from 8k to 44k). Note that I am able to kill 74k mission bosses in explorers just fine with my kehra.
