I've noticed that the achievement diary states that you only have to have started Ratcatchers. Can I complete the diary without access to the Kwuarm or do I need to get up to 54 herblore to finish Ratcatchers to finish the Fally Med diary? You don’t need 54 herb to finish it. You can get the rat poison from the clock tower quest.

Ratcatchers is overhated because of the mansion guard desync. If you just hop a few times you’ll finish the quest quite quickly. IMO the light puzzles and agility RNG in the other quests (run through underground pass again!) is way worse. You want to know the real kicker about mm2.
